School Lunch Program 07-08 
Order Form

Orders: When you come to registration it will take a few minutes to complete a lunch order form for each child.   These are special forms which you will receive at the fee day registration table.  Enclosed is a copy of the lunch order form with pricing.  You might want to go over the lunch choices with your child(ren) before you come to registration.  Keep in mind you are ordering for the whole year.  There is a limited amount of extra food available at the lunches.   Order changes are discouraged after registration.  Before you complete fee day registration, these forms must be checked by one of the lunch coordinators at the lunch order station.  Please take the time to fill the form out completely by filling in all information in all the sections (name, room number and lunch order) even if your child is not ordering the lunch.  Your pink copy of the lunch order form will be verified at the cashier table when you write your check to Parent Club.  Please thank the vendors that work with us on these lunch days because they provide great service and good prices! 

Pizza Day:The lunch consists of a slice of pepperoni, sausage, veggie or cheese pizza, carrots and a dessert.  Additional slices can be ordered.  Pizza is provided by Papa Johns.

Chicken Day: There are 2 choices.   2 chicken legs or 2 chicken strips served with mashed potatoes & gravy, homemade roll and cookie OR 2 chicken legs or 2 chicken strips served with mashed potatoes, GREEN BEANS, roll & cookie.  Extra pieces of chicken, nuggets  & potatoes can be ordered.  Kentucky Fried Chicken will supply chicken this year.

Culver’s Day: This lunch is provided by Culver’s.  Students will have an opportunity to choose from Culver’s hot dog, chicken tenders, hamburger, cheeseburger, or double cheeseburger.  A side salad will be available as well.  Chips and a cookie are included.  Please note: There are no special orders for burgers.  They will be prepared the same way they are in the restaurant.

Subway Day: This lunch consists of a 4” round or 6” cold sub sandwich with cheese and a choice of ham, turkey, or tuna.   We will also offer a 4” or 6” hot BBQ sandwich or a 6” hot meatball sub.    The kids will have the option of adding lettuce, tomato, pickle, mustard or light mayo to their sandwich.  A bag of chips and fresh Subway cookie also comes with this lunch.

Volunteers:  There will be sign up sheets for lunch helpers at the lunch order tables.  If you are a new St. Matthew parent this is a great way to meet the kids, get to know other parents and get a free lunch!!   The time commitment is usually from 11 am to 1:00 pm (this includes clean-up!).  Little ones are welcome too.  You can sign up to help with any of the lunches on a regular basis or substitute for any of the lunches.  The lunches are usually on Thursdays.

Drinks:  PLEASE NOTE:  Drinks are not provided with these lunches.  The students should bring drinks or milk tickets as they do on regular lunch days.

Hot Lunch Dates 2007-2008
NOTE: Special lunch days are on Thursdays!
